Using the information in the case notes, write a referral letter to the genetic counselor, Dr. Rachel Moore, at Metropolis Genetic Counseling Clinic, 123 Genome Drive, Metropolis, requesting an assessment for Mrs. Parker and her husband due to family history of cystic fibrosis.
