Writing Task:

Using the information in the case notes, write a discharge letter to Mrs. Adams’ general practitioner, Dr. Linda Harris, at Lakeside Family Practice, 789 Oak Street, Riverside, outlining the patient’s condition and discharge plan.

In your answer:

  • Expand the relevant notes into complete sentences
  • Do not use note form
  • Use letter format

You are the obstetrician who supervised the following patient’s labor and delivery.

PATIENT DETAILS:

Patient: Mrs. Sarah Adams

Date of Birth: 10/11/1990 (34 years old)

Admission Date: 19/08/2024

Discharge Date: 22/08/2024

Reason for Admission: Labor & delivery

Medical History:

  • Gestational Diabetes (diagnosed at 28 weeks, managed with diet and insulin)
  • Hypothyroidism (diagnosed 5 years ago, managed with levothyroxine 75 mcg daily)
  • Previous miscarriage at 8 weeks (3 years ago)
  • Previous cesarean section (2020) due to breech presentation
  • History of mild depression postpartum after her second child (resolved with counseling)
  • No known allergies
  • Regular prenatal care throughout her latest pregnancy

Social History:

  • Lives with her husband, who works as an engineer, and two children (ages 6 and 4)
  • Works as primary school teacher
  • Non-smoker, occasional social drinker (1-2 glasses of wine per month)
  • Active lifestyle, enjoys yoga and swimming
  • Recently moved to new house closer to her workplace

Labor and Delivery:

  • Admitted in active labor on 19/08/2024
  • Normal vaginal delivery on 20/08/2024 at 3:45 AM
  • Baby girl, birth weight: 3.2 kg, APGAR scores: 8 at 1 minute, 9 at 5 minutes
  • No complications during delivery
  • Spontaneous rupture of membranes 6 hours prior to delivery
  • Labor lasted 12 hours from admission to delivery

Postpartum Course:

  • Stable vitals: BP: 120/80 mmHg, Pulse: 75 bpm, Respiratory Rate: 18 breaths/min, Temp: 36.8°C
  • Mild perineal tear, repaired with sutures
  • No excessive bleeding postpartum
  • Successful breastfeeding initiated, baby latching well
  • Routine postpartum care and education provided
  • Emotional status stable, no signs of postpartum depression
  • Mild constipation treated with lactulose

Discharge Medications:

  • Levothyroxine 75 mcg daily (ongoing for hypothyroidism)
  • Insulin glargine 10 units at night (ongoing for gestational diabetes)
  • Iron supplements 60 mg daily (for mild anemia)
  • Paracetamol 500 mg PRN (for pain)
  • Lactulose 10 ml daily (for constipation)

Discharge Plan:
